๐Ÿ’™ National Donate Life Month ๐Ÿ’š

During this special month,
letโ€™s honor living kidney donors and the power of giving!

Submissions due April 24, 5pm ET

Here’s how:

  1. Get Inspired: Encourage your child to think about the amazing impact living kidney donors have on others.
  2. Create the Art: Let your child’s imagination run wild โ€“ painting, drawing, collage, anything goes! Wholly original, digitally-generated artwork is acceptable, but no AI-generated art, please. One entry per child.
  3. Submit their artwork online. The submission may be a scan or photo of the child’s artwork. Please submit well-lit images without shadows.ย 
  4. Spread the Word: Encourage friends and family to join.
  5. Win Prizes: Their art could be featured on NKDO’s website and socials. Amazon gift cards for winners – $50 for first place in each age category and $25 for second place in each category!

Submit Your Artwork

  • The art contest is open to children in two age categories – 11 years and younger and 12 – 18 years.
  • Parental consent is required for participants under the age of 18.
  • The winner in each category will be announced at 3pm ET on Tuesday, April 30 on NKDO’s Facebook and Instagram page.

Children’s Art Contest Release Form

I, the undersigned parent/legal guardian of the participating child, grant permission for my child, [Child’s Name], to participate in the NKDO Children’s Art Contest, organized by the National Kidney Donation Organization, from now on referred to asย  NKDO.ย 

Release and Permissions:

  1. I grant NKDO the right to use, reproduce, and distribute the submitted artwork for the following purposes:
    • Display or publication in digital and print contest-related materials, including but not limited to NKDO websites, social media, press releases, and brochures.
    • Display or publication in digital and print format for other purposes related to NKDO’s work to inform, educate, and advocate for living kidney donation.
  1. I understand that my child’s artwork may be photographed, recorded, or otherwise documented for purposes related to NKDO’s work to inform, educate, and advocate for living kidney donation and now consent to such activities.
  2. I acknowledge that my child’s first name, age, and city/state/country may be disclosed in conjunction with the display or publication of the artwork, but personal contact information will not be shared publicly.
  3. I affirm that I am the legal parent or guardian of the participating child and have the authority to grant the permissions outlined in this release.

Liability Waiver:
I release and hold harmless NKDO, its officers, employees, agents, and representatives from any claims, damages, or liabilities arising out of or in connection with my child’s participation in the contest, including but not limited to personal injury, property damage, or copyright infringement.

Consent: I have read and understand the terms of this release form and consent to its provisions.
